Methodology: Enzyme immunoassay. Purified GBM antigen (NC1 alpha-3 chain of type IV collagen). IgG specificity.

Indications: Glomerular basement membrane (GBM) antibodies are detected in untreated GBM disease and Goodpasture’s syndrome. Levels of GBM antibodies can be used to monitor response to treatment.

ANA and ANCA should be requested to rule out other forms of renal-pulmonary syndrome and systemic vasculitides.

An on-call service is available for clinically urgent ANCA (MPO and PR3 antibodies) and GBM requests on Saturdays, Sundays and Bank holidays between 9am and 5pm (after 1.30pm, advice only). Reference Ranges:
< 7U/mL = negative
7-10 = equivocal
> 10U/mL = positive
