Internal Laboratory Information
Our RR for theophylline is 10.0 - 20.0 mg/L, and the analytical linearity is up to 40 mg/L. Above 40, the SOP advises diluting with zero calibrator. We occasionally get theo's over 54 and in overdose or toxicity cases, treatment can depend upon the plasma concentration. In the literature, and on the Poisons Unit website, haemofiltration is recommended at levels above 60 mg/L. So there is a clinical need to know an abnormal value. If we dilute values over 40 one in three with the diluent, this gives a measurement range of up to 120 mg/L, which should be sufficient for all cases. As our lower levels are reported as <3, I would suggest a reporting range of <6 to 120 mg/l (M Baines).
